90 Aledade Partners Recognized for Blood Pressure Control Excellence

Federally Qualified Health Centers stand out among Aledade partners that receive American Heart Association and American Medical Association honor

Aledade, the nation’s largest network of independent value-based primary care, today announced that 90 of its primary care practice and community health center partners have been recognized by the American Heart Association and American Medical Association for their exceptional performance in controlling hypertension through the Target: BP™ initiative.

The Target: BP™ initiative, launched by both associations in 2015, recognizes health care organizations that achieve blood pressure control rates of 70% or greater among their adult patient population who have hypertension. According to the Centers for Disease Control and Prevention, about 47% of U.S. adults – or 119.9 million people – have hypertension, which increases the risk for heart disease and stroke, the leading causes of death in the United States.

In the 2024 Medicare Shared Savings Program (MSSP), Aledade Accountable Care Organizations (ACOs) saved Medicare $1 billion by delivering proactive, preventive care. In fact, Aledade partners scored 4% higher than the national average for hypertension control, ensuring that Medicare patients received critical support managing their blood pressure. Aledade partners achieve these results through a comprehensive approach that combines innovative data dashboards, expert hypertension consulting, nurse-led remote patient monitoring and targeted patient outreach to optimize care delivery and engage patients in their care.

Community Health Centers Excel at Blood Pressure Control

Sixty of the 90 recognized Aledade partners are Federally Qualified Health Centers (FQHCs) or community health centers (CHCs). FQHCs serve medically under-resourced areas, including rural areas and populations, and provide primary care services regardless of the individual’s ability to pay. In the 2024 MSSP, Aledade’s 10 CHC-only ACOs achieved over $585 in savings per beneficiary and earned a total of more than $96 million in shared savings.

Aledade’s Community Health Center Center of Excellence (CoE) fosters collaboration and best practice sharing among health centers nationwide, utilizing blood pressure data insights from Aledade's proprietary platform. For example, the CoE helps centers in Mississippi and California accurately assess uncontrolled blood pressure by taking two or more in-clinic readings and factoring in patient-reported at-home readings. This larger data set helps clinicians identify patients needing more support.
